Featured in the News: Interactive Metronome Therapy Helps Amputees Learn to Walk Again
When Fred Davis lost both of his legs to infection –he thought his walking days were over.Then he was introduced to a therapy that combines movement and timing to help the brain redevelop motor skills. It’s called Interactive Metronome. For six months, Mederi Caretender therapist Tameka Walker has been helping Davis relearn to walk. "Left hand, right toe, left hand, right toe. Got it?” she instructs. To the chime of a cowbell, Davis steps one foot forward on a mat – and then pulls it back. Then, it’s the other foot, always sticking to the beat. If his rhythm is off, he hears an unpleasant buzz. Davis suffers from diabetes. In 2005 — he injured his toe. The untreated injury led to gangrene and an eventual amputation. In February of this year, another injury led to the loss of his right leg.

The type of clothing you wear, the temperature, the foods you eat, your daily schedule, where you play, the list could go on and on. These changes can put our sensory kiddo’s over the edge. But, there are things you can do to help!!
Sports and IM
Several of the teens that I see have come into the clinic for learning issues, but also are wondering if IM will help them in sports. Whenever a child brings this up to me, I really like to involve them in creating an exercise that they think will help them to work on their specific sport. Now sometimes, the activity is very crazy, and the child is unable to perform it – but sometimes they come up with something really good that I then ask their permission to use with other children.

The important of timing in Speed Skating and the use of the Interactive Metronome
Researchers at Korea University College of Medicine (Park et al, 2012) recently conducted a neural imaging study of elite speed skaters to investigate whether training of complex motor skills resulted in structural changes to the cerebellum. The cerebellum responds to intense, repetitive training with increased brain mass in areas critical for skilled motor movement, in this case for control of balance, precisely coordinated movement, and visually guided movement. The authors compared the cerebellums of professional speed skaters to individuals who did not engage in regular exercise. They found that the specific skills required for speed skating that were trained repetitively resulted in structural changes to the brain that enhanced balance and coordination. They also found that the particular side of the cerebellum that was exercised repeatedly was affected (i.e., the right side due to maintaining balance on the right foot during turns). Of note, the cerebellum is also a central part of the brain’s internal timing network. The timing and synchronization of neural signals ultimately controls balance and coordination…

I have found in my area (Northeastern part of the country), that these children seem to fall somewhere between those who are diagnosed with Autism, and those diagnosed with Aspergers Syndrome. So their responses to using the Interactive Metronome have been very similar, with great progress in a shorter amount of time…

Hey Kids! Today was my Lucky Day
Some of my patients are very sports oriented. When we start IM, I tell them all about how professional sports teams such as the Miami Dolphins or the golf pro ViJay Singh use IM as part of their training to improve and enhance their performance. Every once in a while, I get a patient who just really “gets on a roll” and makes improvements very quickly earning lots of “bursts” (so many consecutive super right on the beat hits).
Bill was one of those kids. He just loved beating his scores during every visit. He got the idea in his head, that he wanted to reach 1000 bursts before he completed his sessions. Every visit, Bill would request to perform various exercise that he knew he was good at, just to increase his number of bursts so he could reach his goal. Now a kiddo (who happened to have Asperger’s syndrome and ADHD) who has set his mind to something is very difficult to stop!…
A few safety rules for children with ADHD
These safety rules for ADHD children may seem obvious, but we find that reminders seldom hurt — especially for caregivers who don't live every day with attention deficit and hyperactivity.
A parent or older sibling must always supervise free play.
Stop his jumping or running sooner than you might with another child.
When outside, hold hands.
When out for a stroll, walk on the side that's closer to the street.
Be ready to stop a child with your actions, not words, should he happen to dart off.
Hold hands in stores or malls, or place the child in a wagon or cart with a seat belt.

Hemiplegic Cerebral Palsy
One of my favorite groups of patients to work with using Interactive Metronome® are those affected by hemiplegia. These are my kiddos who either totally neglect one of their arms, or who only use their arm when absolutely necessary. They constantly hear “use both hands” or “use your helper hand” time and time again. There are other options for those affected with hemiplegia such as constraint-induced therapy, which can also be beneficial, but IM is a great way to get them to start using their affected upper extremity.
